STEP 2: Check-in with our registration staff inside the Emergency Department main entrance, facing Logan Street, on the date of your scheduled service to complete final steps of the registration process. Please be advised that you will sign all consent forms during your hospital visit. To expedite these final registration steps (and to meet the requirements of your insurance company, Medicare, and Medicaid), please bring the following items with you to the hospital: - Valid state or federally issued photo ID like a driver's license, government ID or passport
- Your insurance card, including your policy number and billing address
- Your Social Security card
- A list of current medications and known allergies
- Any forms or papers given to you by your doctor
- A list of any previous surgeries and hospitalizations
If your healthcare is covered by a worker's compensation claim, please bring the accident date, insurance carrier, where to mail the claim and the name of your employer.
